Initial screening criteria for access

You are qualified to request assay service from us if any of the following apply-
You are requesting Assay Services for an NIH research funded project
Your request is directly related to the NIH "Healthy People 2010" priorities
Your request is biomedically relevant to the WNPRC Mission Statement

Once you have satisfied the NIH requirements for access to Assay Services, you need to comply with the following conditions of use:

(1) forward a copy of any publication arising from assay work using our facilities to Dr. Toni Ziegler, Wisconsin National Primate Research Center, 1223 Capitol Court, Madison, WI 53715-1299, USA,
(2) acknowledge the involvement of WNPRC Assay Services and the partial support of NIH grant RR000167 in any publication arising from assay work using our facilities, and
(3) if we are involved in assay development and/or validation work for your project, co-authorship is required for the appropriate WNPRC Assay Services personnel.

PLEASE NOTE: Your use of WNPRC Assay Services provides your agreement to these three conditions of service use.

...before you request your first assay service.

STEP 2. Please provide this information to your purchasing agent in order to establish an account with the University of Wisconsin National Primate Research Center (WNPRC).

An internal UW-Madison customer may elect to have charges direct billed. If you prefer this method please e-mail me your funding string including the project/grant and DDS numbers, for example: 144-AB12 34-8800. Indicate a beginning and ending date that this project/grant may be charged.

If you are part of the University of Wisconsin System and will be using funds administered through the UW-Madison please substitute the phrase "Internal Work Order" for Purchase Order.

A purchase order, written in U.S. funds needs to be issued to the Wisconsin National Primate Research Center. It is recommended that your purchase order be a blanket order, which is valid for services over a range of time indicating a beginning and ending date; rather than one specific date. This is due to the fact that another purchase order and new WNPRC account would have to be generated if additional services are required.

The blanket order must include address information for submission of invoices, the name, e-mail address, and phone number of the individual we would contact to provide your new WNPRC account number. The official vendor copy of the purchase order may be mailed or faxed to me. Upon receipt of a valid purchase order an account number will be established. I will then contact the person specified on the purchase order with your WNPRC account number. The WNPRC account number must be included when submitting any samples or requests for service. It is the responsibility of the account holder to ensure that you do not exceed the dollar amount indicated on the purchase order. Please do not send us your samples until you have first secured a valid Primate Center account number. We can not do any assay work without that valid number.

Payment is due upon receipt of invoice. Checks must be payable in U.S. funds, drawn on a U.S. bank and issued to: Wisconsin National Primate Research Center. Related Information: The University of Wisconsin's Tax Identification number is: 39-1805963

STEP 3. Fill out our Service Request Form on the Web and submit it to us.

STEP 4. Please include a printout of the completed Service Request Form with your samples when you ship them to us so that we can properly identify your samples and the procedures you have requested.

STEP 5. Send to us (as an e-mail attachment) a spreadsheet file containing your sample code information.

When can you expect RESULTS? Usually within 2-3 weeks of receipt of samples.
